Procedure of a Hair Transplant
A well-thought-out schedule for the treatment is essential. In the example below outlining the procedure for a treatment day, with up to 750 grafts, every minute is planned. For larger hair transplantation areas ranging from 1500 to 3000 grafts, the procedures are adjusted to provide an even more comfortable experience for the patients.
We adhere to a predefined schedule for every procedure. Experience has shown that a well-structured timeline is a crucial factor for a successful and effective hair transplant using our method.

Marking of the transplant areas
The hairlines or other areas are marked for treatment, and the donor areas are shaved.
The decision on how the facial frame will ultimately look is slept on overnight.
On the treatment day itself, any changes to the hairlines or other areas can be discussed again.
All questions will be answered thoroughly.
Treatment Day
During the first half of the day, the hair follicles to be transplanted, also called grafts, are harvested. On average, depending on individual circumstances and conditions, you can expect to harvest approximately 750 grafts per day.
Depending on the condition of the donor area, such as in repair work, the number of grafts may vary even within the same time frame.
The grafts are placed in a special solution, maintaining a temperature between 3 and 5 degrees Celsius without interrupting the cold chain. This allows us to theoretically store the harvested grafts for several weeks. Our dedicated operating rooms are equipped to monitor the grafts technically 24 hours a day.
In addition to the extraction of the hair follicles and the number of grafts to be harvested, another important factor is selecting the grafts that are suitable for achieving optimal results.
This not only increases the number of hairs responsible for the density illusion but also leads to a more natural outcome. For example, fine hairs must be transplanted into the hairline or temple area to enhance the natural appearance.
We sort the hair follicles according to their characteristics and location to choose the ideal position.
The optimal graft (number of hair follicles or hairs growing from one graft) is as follows:
- Temples/Eyebrows: 1-hair graft (very fine, 1 very thin hair)
- Hairline: 1-hair graft (fine, 1 thin hair)
- Behind the hairline: 1-hair or 2-hair graft (1 thicker hair or grafts with 2 hair follicles)
- Behind or in the crown area: 3-hair, 4-hair, 5-hair grafts, etc. (grafts with 3 or more hair follicles)
After the lunch break in the second half of the day, the micro-slits are created, and the harvested grafts are then inserted into the openings. Depending on the patient, scheduled breaks are included, and this process can take several hours. Under optimal conditions, a more time-efficient execution may also be possible.
Hair Growth Direction / Angle and Slit Size in Hair Transplantation
For the practitioner, it would be easiest to insert the grafts at a 90-degree angle. This allows the harvested grafts to be implanted in a few hours using larger slits, without the need for magnification or with the assistance of simple magnifying glasses.
It becomes more challenging when the slits are smaller or at most the same size as the grafts and are opened in their original growth direction, considering the angle of fall.
These slits need to be opened so that, just like before hair loss, the original growth direction and identical fall angle can be observed and achieved. This is the “king’s discipline” and can only be accomplished with an appropriate binocular loupe (e.g., with 5x magnification).
The two following details are crucial:
- Precise slit positioning in the natural growth direction
- The smallest possible slit size
The smaller size of the micro-slits has the advantage that after insertion, the grafts tightly seal these openings, like a cork in a bottle, completely stopping any potential bleeding.
As a result, the grafts do not have the opportunity to move unintentionally (avoiding “popping”). The growth direction and fall angle determined during the treatment are no longer left to chance due to crust formation and other factors that occur during the healing process. Consequently, the hairs can fall according to the natural growth direction after subsequent growth, ultimately enhancing the illusion of density.
It should be noted once again that achieving the original density is not possible with a single hair transplant, especially when restoring a swirl (e.g., at the crown) or completely bald areas.
With this method, it is possible to extract grafts multiple times from the same donor area, with minimum intervals of 6 months.
- Using the maximum amount of material, for example, 1/3 of the head (or the crown) can be treated (3-4 treatment days).
- For receding hairlines with temple areas, 2-3 treatment days are required.
- Repair procedures may take longer depending on the extent, but typically no more than 4 days.
- Treating scar tissue typically takes half a treatment day but may vary depending on size and scope.
